1.    Fill out W-9 and other forms at SABO meeting and return to Allen.

    2.    Pay SABO Dues: $45 to cashapp: $sabo2020

    3.    Register with FHSAA: FHSAA - Default

    4.    Register for Jessica Lunford Act: $83.75 (good for 5 years)

    5.    Purchase uniform: Purchase Officials Supplies

    6.    Get books: officials manual, rule book, case book, copy of rule changes

    7.    Take rules test (website + deadline window)

etc.) in SABO for help or to review answers before submitting.

        *    Must pass with a 75% or higher.

FHSAA MISCELLANEOUS INCIDENT REPORT FORM - AT6 U 

This form must be submitted within 24 hours of an incident to the FHSAA.  Report miscellaneous situation: facilities issues, event cancellation, injuries, spectator misconduct, equipment/uniform compliances or any other incident not previously mentioned.  

UNSPORTSMANLIKE CONDUCT INCIDENT REPORT FORM - AT6

 This form must be submitted within 24 hours of an incident to the FHSAA. Use this form to report unsportsmanlike conduct of student/coach/school representative.
